Apple is now selling unlocked iPhone 5 models in the US. Unfortunately, only the GSM model will be unlocked and are available for $649/$749/$849 for the 16/32/64 GB models.
According to the company website:

To start using it, simply insert the card into the slot on your iPhone and turn it on by pressing and holding the On/Off button for a few seconds. I believe another smart move Apple’s part is to provide a nice financing deal, which enable customers to purchase the iPhone now and pay no credit for up to 18 months. This would be great for those looking to buy an iPhone but can’t afford to shell out for the $649 upfront.
